Do not resusitate orders:

Patients/health care agents who notify Island Ambulatory Surgery Center, LLC (IASC) of their desire that a DNR order be in effect at IASC will be referred to Administrator. If an adult patient with capacity or a health care agent requests a DNR order, they will be informed that DNR’s are NOT honored at Island Ambulatory Surgery Center, LLC.

If a patient brings signed, executed DNR to the surgery center, it will be maintained in the chart to be transferred in the event the patient is sent to a hospital for further care. Otherwise, DNR’s will not be recognized or followed at Island Ambulatory Surgery Center, LLC.


Health Care Proxy:

IASC will supply patients with copies of the New York State’s “Health Care Proxy” and a member of IASC Administration will meet with the patient and/or his/her agent to discuss the proxy if requested by the patient.


Living Wills:

All Advance Directives, other than the Health Care Proxy and a DNR order, will be referred to IASC administration in order to determine whether the Advance Directive has been expressed in a clear and concise manner. Upon admission to IASC, each patient will be asked if they have a Health Care Proxy or wish to have one.

If the patient indicates that he/she has executed or wishes to execute a Health Care Proxy, or the patient has provided written instruction about treatment, the Receptionist or Nurse will place a copy of the executed health care proxy or written instructions on the chart.

In accordance with New York State’s “Patient Self-Determination Act”, information pertaining to Patient’s Rights and Responsibilities and Advance Directives is given to patients in advance of the day of surgery by the referring surgeon.

Patients need to be aware that filling out an Advance Directive is entirely voluntary. The appointment of a Health Care Proxy is a serious decision. Patients are encouraged to consider it well in advance of the scheduled surgery and discuss it with their physician.
